Forms

This portion of my portfolio includes important forms and worksheets that relate to my progress through the doctoral program at the FSU School of Library and Information Studies. Most of these forms are password protected because they contain my FSUSN, a unique identification number which I desire to keep private. Print copies of these forms are available upon request. The forms include:

• An updated PhD Program of Study (formerly known as the Doctoral Program Worksheet) with my current and proposed plan of study, as well as grades earned in courses I have completed.
• A copy of my PhD Supervisory Committee Form reflecting my current committee. A copy of the form signed by those members and the Director of SLIS to constitute my current committee is also available.
• A copy of my Faculty Readiness Development Plan for the Preparing Future Faculty (PFF) program, indicating the progress I have made so far and my intended plans to complete the program. You may also view the original plan as signed and endorsed by my major professor / faculty advisor.
• A copy of the PhD Annual Review Signature Form that should be used by my committee to complete my annual review based on this portfolio and a face-to-face meeting.
